Abses Submandibula Odontogenik pada Penderita Idiopatik Trombositopeni Purpura di RSUP Dr. Sardjito

Abstract


Idiopatik Trombositopeni Purpura (ITP) adalah kelainan yang berhubungan dengan penurunan jumlah platelet yang beredar dalam plasma darah yang dapat disebabkan oleh peningkatan destruksi platelet karena autoimun. Penurunan jumlah platelet akan menurunkan kemampuan hemostasis tubuh. Prevalensi ITP adalah 4 sampai 5,3 per 100.000 anak, dengan tingkat mortalitas ITP kronis sekitar 4%. Abses submandibula menempati urutan pertama abses leher yang paling sering dijumpai (42,30%) dengan prevalensi causa odontogenik sebesar 34,21%. Tujuan laporan kasus ini adalah untuk mempresentasikan keberhasilan evakuasi pus dan eliminasi gigi kausa pada kasus abses submandibula odontogenik pada seorang anak penderita ITP. Seorang anak perempuan berusia14 tahun penderita ITP dengan riwayat sakit gigi geraham kanan bawah dan pembengkakan pada submandibula kanan dengan fistula ekstra oral yang mengeluarkan darah dan pus datang ke IGD RSUP Dr. Sardjito dengan kondisi lemah. Kasus ini dirawat bersama dengan bagian hematologi onkologi anak untuk penanganan kondisi trombositopenia sehingga memungkinkan untuk dilakukan tindakan incisi drainase dan pencabutan gigi kausa. Tindakan perawatan gigi dan pembedahan dapat dilakukan pada penderita ITP dengan memperhatikan angka trombosit. Untuk mencapai angka trombosit yang cukup, diperlukan kerja sama dengan dokter bagian hematologi, sehingga resiko perdarahan durante dan pasca tindakan dapat di minimalisir

ABSTRACT: Odontogenic submandibular abscess in patient with Idiopathic Trombocytophenia Purpura at General Hospital Centre Dr. Sardjito. Idiopathic Thrombocytopenia Purpura (ITP) is a disorder associated with a number decreasing of platelets circulating in the blood which can be caused by platelet destruction increasing due to autoimmune. Low platelet count will decrease the body's ability to hemostasis. The prevalence of ITP is 4 to 5.3 per 100,000 children, with a mortality rate of approximately 4% of chronic ITP. Submandibular abscess is the first ranks of neck abscesses which are the most common (42.30%) with a prevalence of 34.21% odontogenic cause. The purpose of this case report is to present the early success of the evacuation of pus and elimination of causative tooth in the case of odontogenic submandibular abscess in a child with ITP. A 14 year old girl diagnosed with ITP and a history of right lower molar tooth pain and swelling in the right submandibular with extra-oral fistula that blood and pus came to the ER department of DR.Sardjito feebly. This case was treated together with pediatric department of hematology oncology for thrombocytopenia conditions allowing for incision drainage and extraction of causes tooth. Dental treatment and surgery can be performed on patients with ITP with regard platelet numbers. To achieve a sufficient number of platelets, it is neccessary to cooperate with hematologist, so the risk of bleeding during and following the surgery can be minimized.
